Intake and Outreach Services
Hui Malama provides intake and outreach services to clients in need of special or individualized assistance. Our Outreach Workers provide support to clients within their homes,or at a Hui Malama facility.
Medication Management
Services include:
• Assistance with prescription authorizations
• Arrange medication pickups and refills
• Monitor medication amounts and schedules
• Prepour medication into separate doses
Individual Services
Services include:
• Blood pressure screening
• Glucose screening
• Assist with forms and applications. IE; DHHS, SSI, etc.
• Schedule appoitments and screenings for clients
• Do follow ups with Primary Care Physicians
